Please follow below steps to create annotation report in crm 2011.
1. Go to Reports area in crm 2011.
2. Create "New" report.
3. Click on the "Report Wizard" button.
4. Enter Report Name is "Annotation List".
5. Choose the "Notes" option in the 'Primary record type' drop down list.
6. Then Click on "next" button.
7. Choose "All Notes" option in 'Use Saved View' drop down list and click on "Next" button.
8. Select fields which we need to show in the report and click on "Next" button.
9. Select "Table only" radio button option and next-->next--> finish.
Now we can run the report.Here we can get all annotations in CRM.
